In my practice I am fortunate in that I own a Surgery Center and we do the vast majority of our prostate biopsies with I.V. sedation in which the patients are asleep. If you don’t have this option you can at the time of the scheduling, you should ask for pre-medication to make the procedure more comfortable.
You also want to ask about doing a rectal culture before the biopsy to guide the antibiotic choice to prevent an infection.
